2. Paper Expectations of Participants Form

Adults Must Complete the Following Forms:

1. Paper Liability Release Form
2. Paper Chaperone Guidelines Form

Priests/Deacons Must Complete the Following Forms

1. Paper Liability Release Form
2. Paper Chaperone Guidelines Form
3. Letter of Good Standing from your Diocese

All of these forms can be assembled alphabetical and separated by group in a master binder to be turned in at registration.
