Steubenville East 2013 will take place July 19-21, 2013 at the University of Rhode Island. Registration is currently full, but click here to register for the waiting list.

Once your group has registered, please complete these forms for each participant:

Teens Must Complete the Following Forms:

  1. Paper Liability Release Form
  2. Paper Expectations of Participant Form

Adults Must Complete the Following Forms:

  1. Paper Liability Release Form
  2. Paper Chaperones Guidelines Form
  3. Paper Safe Environment Compliance Form*

Priests/Deacons Must Complete the Following Forms:

  1. Paper Liability Release Form
  2. Paper Chaperones Guidelines Form
  3. Priest Aptitude Certificate OR Deacon Aptitude Certificate

All of these forms can be assembled alphabetical and separated by group in a master binder to be turned in at registration. The Safe Environment Compliance Form, as indicated on the form, must be turned in ahead of time.
