Once your group has registered, please complete these forms for each participant:

Teens Must Complete the Following Forms:

1. Paper Liability Release Form
2. Paper Expectations of Participants Form

Adults Must Complete the Following Forms:

1. Paper Liability Release Form
2. Paper Chaperones Guidelines Form
3. Paper Safe Environment Verification Letter by Pastor

Priests/Deacons Must Complete the Following Forms:

1. Paper Liability Release Form
2. Paper Chaperones Guidelines Form
3. Priest OR Deacon Statement of Ordinary Provincial*

*All priests attending the conference with your group must have Faculties approved through the Archdiocese of Atlanta prior to the conference.
Fill out the Statement of Ordinary and mail to:

Once granted we are given a copy, but bring a copy with you just in case.

All of these forms can be assembled alphabetical and separated by group in a master binder to be turned in at registration. The Statement of Ordinary Provincial, as indicated on the form, must be turned in ahead of time.
